How to link community names to addresses

I have a list of community names that I would want to link to specific corresponding addresses. The specific addresses already exist in Knowledge Graph under various entities (for example, providers and locations). What do you recommend as the best way to approach this?

The end goal is when a user includes one of these community names within a query, it would return the same results as if they had entered the corresponding address

Circling back to provide a summary of how this was resolved, in case others are in a similar situation. We were able to use a Geomodifier field with the community names as an NLP filter and prioritize the field using nlpFilterOrder for provider and location entities.
